School Library Media Administration Application Process

Steps to enroll online

1. Go to http://www.usu.edu/admissions/applyonline
2. Read through the "essential steps"
3. When you have read through the steps, click on "Online Application"
4. Click to create a new account
5. Create a login ID and password
6. Choose either Undergraduate Returning or Undergraduate Transfer
   
   1. If you have attended USU formally you are applying as a "returning" student. If you haven't attended USU formally, you are considered a "transfer" student.
   2. Even if you have graduated, you want to choose "Undergraduate" if you are only seeking an endorsement. If you are seeking an advanced degree then click on "School of Graduate Studies."
7. Click continue
8. Fill out your name and which term you would like to be admitted and click "Fill Out Application"
9. Fill out each part of the form by clicking on each section.
10. Checklist item 11 of 11. In the box, "additional comments you consider pertinent to this application" please state which endorsement you are seeking.
11. If you are unable to complete it immediately, you will be able to return using your username and PIN to finish the form.
12. On the "Enrollment Information" you must state your intent. Use the drop down box and select "Post Graduate Endorsement/Certification.
13. Once you are finished click "Application is complete."
